What is Home Care?

Home care—also known as home-based care or personal care—refers to a wide range of care and support services delivered in a person's own home. YDCare also provides condition-specific care at home for patients with cancer, chronic obstructive pulmonary disease (COPD), stroke, and other complex health needs.

Who is Home Care suitable for?

Home healthcare services are available to anyone who needs care and support at home, with no restrictions on age, gender, or location. You may benefit from home care if you have limited mobility or difficulty leaving home and if any of the following apply:
1. Post-discharge or recovery care: Recently discharged from hospital or a care facility following illness, injury, or surgery.
2. Frequent medical visits: Requiring frequent clinic or emergency visits for care needs, affecting work efficiency and daily life planning.
3. Long-term condition management: Managing one or more chronic health conditions.
4. Medication management needs: Taking multiple medications and requiring support to ensure correct use or having recently changed prescriptions that need monitoring for side effects and effectiveness.

What is the difference between home care and outpatient care?

The main differences lie in the care setting and the people they are best suited for. Home care is delivered by healthcare professionals in the patient's home and is ideal for individuals with limited mobility or long-term care needs, such as older adults or people with chronic conditions. It allows patients to receive care in a familiar environment while reducing the burden of travel. Outpatient care requires patients to attend a clinic or hospital and is more appropriate for situations involving specialised equipment or specialist treatment. The most suitable option should be discussed with a doctor based on individual health needs.

Who decides which type of care professional will come to my home?

Unlike many providers, YDCare has a dedicated Nurse Manager role, filled by Hong Kong–registered nurses. The Nurse Manager communicates with you one-to-one to conduct an initial assessment and provide a quotation, develops a personalised care plan, matches the most appropriate care professionals, and prepares a list of required medical equipment and care supplies—ensuring professional oversight from planning to delivery.
